What Inlays, Onlays & Overlays Help With
These restorations may be considered when damage is too extensive or structurally demanding for an appropriate direct filling, while sufficient healthy tooth structure remains for an alternative to full crown coverage.
Possible situations include moderate or larger decay, large fillings requiring replacement, worn or damaged restorations, weakened cusps, loss of tooth structure and selected cracks or fractures. The condition of the individual tooth determines which treatment should be considered.
Inlay, Onlay or Overlay?
An inlay generally fits within the biting surface between the tooth's cusps and may be considered when those cusps do not require coverage.
An onlay extends over one or more cusps when part of the chewing surface requires additional coverage. An overlay provides more extensive coverage across the chewing surface and cusps while retaining suitable remaining tooth structure.
The choice cannot be made from restoration size alone. Your dentist also considers the location of damage, cracks where present, remaining tooth structure, bite and overall condition of the tooth.

What Determines the Right Restoration?
The goal is not simply to choose the option that covers the least tooth structure. Treatment needs to provide appropriate coverage for the condition and restorative needs of the tooth.
Material and restoration design may be influenced by the tooth's location, remaining structure, bite and chewing forces, size of the restoration and aesthetic requirements. For cracked teeth, the position and extent of the crack and the condition of the dental pulp may also affect treatment.
